About this talk
Introduction: Patients with lupus erythematous may develop an acute eruption clinically similar to toxic
epidermal necrolysis or erythema multiforme. Here we reported a 12 year-old female patient who had systemic lupus erythematous developed erythema multiforme-like lesions
Case Report: 12 year old female presented to the OPD with chief complaints of fever with target lesions over both the palms and soles along with hemorrhagic crusts over the lips. Clinically diagnosed as erythema multiforme and treated with oral corticosteroids. Three days into the treatment, she complaints of exacerbation of lesions with involvement of face and oral cavity which led us to evaluate the patient for connective tissue disorder.
On examination -multiple target lesions over bilateral palms and soles along with face. Oral cavity - multiple ulcers over the buccal and palatal mucosa. Lab investigations showed - Hb-10.7gm/dl, ANA -positive (>12.0). Histopathology: -epidermis shows hyperkeratosis with acanthosis. Basal layer thickening with vacuolar degeneration. Dermis shows pigment incontinence with perivascular infiltration and dermal edema conclusive of SLE.
Conclusions: Childhood SLE in itself is a rare disease and this case presented with erythema multiforme
like lesion is a rarity.
